If we are unable to contact your patient, or if they decline to make the appointment, we will notify you.
Once the appointment is complete, you will receive Dr Shami's clinical letter within the week.
Please let us know if your patient requires urgent attention, by marking the referral as urgent, or contacting the rooms directly.

GP Referrals are valid for 12 months from the date of issue. They are then valid for 12 months from the date of your initial appointment, which is considered the start date. You will need to obtain a new referral once the original referral expires, in order to be eligible for the Medicare rebate.
Specialist Referrals are valid for 3 months from the date of issue. They are then valid for 3 months from the date of your initial appointment, which is considered the start date. You will need to obtain a new referral once the original referral expires, in order to be eligible for the Medicare rebate. If you are being referred by a specialist, we encourage you to also obtain a referral from your GP to ensure you are eligible for the Medicare Rebate, should you need care beyond 3 months.
Please obtain a NEW referral to see Dr Shami from your GP or specialist.
If the referral is addressed to another gynaecologist, this is okay.
Forward the referral to us via email, fax or the contact page. We will send you a Welcome Letter to acknowledge receipt.
In addition to the referral, please complete our New Patient Registration form.
This is vital to our triage process.
Be sure to fill it completely & with detail.
Only after it is completed, will we offer you an appointment.

Dr Shami's clinical letter will be sent to your referring GP, and to your chosen Health Care Team.
